LIMITED CONSENT AND ACKNOWLEDGMENT FORM

I understand and hereby consent to CoreClarity®, Inc.'s use of my Clifton StrengthsFinder® assessment results for the CoreClarity In-Powering People Program in order to promote understanding of my individual and/or our group talents. In addition, I understand and agree that my results may be shared with other members of this training program and/or my organization, and hereby consent to use of my results in that manner.

I further understand and give my consent to allow CoreClarity, Inc. to utilize my assessment results, but not my identity, for statistical purposes and/or teaching outside of this training program and/or my organization.

Intellectual Property Acknowledgment:

I acknowledge and understand that CoreClarity, Inc. is not affiliated with The Gallup Organization ("Gallup'') and that Gallup owns all right, title, and interest in all of its trademarks and copyrights relating to Clifton StrengthsFinder®, Gallup®, StrengthsFinder®, and the 34 Clifton StrengthsFinder® theme names. 

I also acknowledge and understand the non-Gallup information I am receiving has not been approved and is not sanctioned or endorsed by Gallup. I acknowledge and understand that the opinions, views and interpretations of my assessment results centered on Clifton StrengthsFinder® are solely the opinions of CoreClarity, Inc.
